Faith Hospice brings support, care and comfort to the families we serve.

Our care is delivered through a highly qualified interdisciplinary team of physicians, skilled nurses, certified nursing assistants, chaplains, social workers and volunteers. This team approach provides families with an invaluable resource, providing a truly encompassing level of services that supports physical, spiritual, social and emotional needs.

Moreover, since hospice is about the quality of life, Faith Hospice strives to support care plans that maintain patients’ ability to remain in their homes, where they have the loving support of their family and friends. As the definition of “home” may differ from patient to patient, our services extend to those residing in long-term care facilities, including nursing homes and assisted living facilities. We can even help with the transition into different settings as needed when a higher level of care is required.

Our expertise in pain control and symptom management enhances the patient’s ability to make the most of life by keeping patients as PAIN FREE AND COMFORTABLE as possible. We remain just a phone call away through our hospice team, which is available 24 hours a day, seven days a week.

Family support is key. Our services enhance the family’s ability to keep their loved one in the comfort of their own home. Certified hospice aides assist with daily tasks such as bathing, light household chores and meal preparation.

Additional services Faith Hospice provides, when needed, include:

  • All hospice-related medications are delivered right to your door
  • All hospice-related medical equipment and supplies are delivered right to your door
  • Patient care education and training for caregivers and families
  • Specialized programs for nursing home and assisted living facility residents
